Esophageal High Resolution Manometry (HRM)

At Advanced Center for Gastroenterology and Liver Disease, Sukedhara, Kathmandu, we provide advanced diagnostic testing including esophageal high resolution manometry to evaluate swallowing disorders. We are the first private center with the most experience in this test in Nepal. Esophageal High Resolution Manometry (HRM) is a specialized test that measures the pressure and movement (motility) of the food pipe (esophagus) and the function of the lower esophageal sphincter (LES).

Why is it done?

  • Difficulty swallowing (dysphagia)
  • Chest pain not related to the heart
  • Persistent reflux symptoms not responding to treatment
  • Suspected motility disorders (e.g., achalasia)
  • Evaluation before anti-reflux surgery

How is the procedure performed?

A thin, soft catheter is gently passed through your nose into the esophagus and stomach. You will be asked to swallow small amounts of water during the test. The machine records pressure changes and coordination of esophageal muscles. The procedure usually takes about 20–30 minutes.

Is it painful?

The procedure is not painful but may feel uncomfortable. You may experience mild gagging or nasal discomfort during insertion.

How should I prepare?

  • Do not eat or drink for at least 6 hours before the test
  • Inform your doctor about medications (some may need to be stopped temporarily)
  • Avoid smoking before the procedure

What happens after the procedure?

  • You can resume normal diet and activities immediately
  • Mild throat or nasal discomfort may occur briefly

Is it safe?

HRM is a very safe procedure with minimal risk. Complications are extremely rare.

  • Achalasia
  • Esophageal spasm
  • Ineffective esophageal motility
  • LES dysfunction
