Hydrogen Breath Test (SIBO Test)

At Advanced Center for Gastroenterology and Liver Disease, Sukedhara, Kathmandu, we offer Hydrogen Breath Test to evaluate patients with bloating, diarrhea, and suspected carbohydrate intolerance or bacterial overgrowth. We are currently the only center in Nepal offering this test. The Hydrogen Breath Test is a simple, non-invasive test that measures hydrogen gas in your breath to assess how well your digestive system is processing certain sugars.

Why is it done?

  • Persistent bloating or gas
  • Chronic diarrhea
  • Suspected lactose intolerance
  • Suspected fructose intolerance
  • Evaluation for small intestinal bacterial overgrowth (SIBO)

How does the test work?

Normally, sugars are absorbed in the small intestine. If they are not properly absorbed, bacteria break them down and produce hydrogen gas. This hydrogen enters the bloodstream and is exhaled in your breath, where it can be measured.

How is the procedure performed?

  • A baseline breath sample is collected
  • You will drink a specific sugar solution (such as lactose, glucose, or fructose)
  • Breath samples are collected at regular intervals (every 15–20 minutes)
  • The test usually lasts 2–3 hours
  • The samples are analyzed for hydrogen levels

Is it painful?

No. This is a completely painless and non-invasive test.

How should I prepare?

  • Follow a special diet the day before the test (low fermentable diet)
  • Do not eat or drink for 8–12 hours before the test
  • Avoid antibiotics for 2–4 weeks before the test
  • Avoid probiotics and laxatives as advised
  • Do not smoke or exercise before and during the test

What happens after the test?

  • You can resume normal diet and activities
  • Your doctor will interpret the results and guide further treatment

Is it safe?

Yes. The test is very safe with no significant risks.
